An example of truly integrated community care!

On September 22, 2011, 911 and the Everett Fire Department received a call where a couple was in a crisis situation. The man had fallen off his roof and the wife was blind and needed care while her husband was taken to the hospital. The fire Department contacted Adult Protective Services who then began looking for an agency to go to this couples home to care for the wife. After calling several agencies for help, APS directed 911 to CareFore, an in home caregiving agency. Within 30 minutes, a caregiver was at the couple’s home. CareForce worked with the Fire Department, 911 and a local hospital social worker, to create a plan of care for the wife. Within this team it was soon discovered that the wife was also in the middle stages of dementia and was considerably agitated. Working together, all entities, including our clinician staff, were able to ensure successful intervention and a proper plan of care for the couple.

CareFore would like to take this opportunity to thank all of those agencies involved in this situation. The Everett Fire Department, 911, Adult Protective Services and the Snohomish county hospital ER. Working as a team, we were able to provide vital care for this couple.
