Co-owner, Susan ‘Sam’ Miller, MN, RN, ARNP, Director of Clinical Services is known locally for promoting stronger communication between hospitals and home care to achieve safer transitions and better care outcomes. Sam recognized the need for safer transitions in the community and developed CarePack, custom services to assist clients transition from the hospital, nursing home or other skilled setting to home.
CarePack Services are designed to assist in the education of the client, reconcile medication and ensure that follow-up medical appointments are made and kept. In addition, a nurse assessment with home health aide assistance can help the client adjust to their new status post hospitalization and incorporate any new health or illness management into their lives.
Any CarePack can be tailored to meet specific client needs, services are bundled and priced accordingly. CarePack Services are packaged in a manner to meet different levels of care and can be selected A La Cart:

RN/MSW Care Management Services
Each service is scheduled for one week, at the end of the week the admitting RN gathers the assessement and intervention data and calls the client and/or family to report the care outcome. The client/family can then decide to choose more care, discuss what outcomes were achieved or discharge from CareForce services.
CarePack is private pay and not meant to replace Medicare home health services, it is an option for those persons who do not qualify for Medicare based on health or lack of homebound status.
