RN Direct Care Supervisor/ Geriatric Care Manager

Ready to work for a company that sets industry standards for in-home care excellence? Then come join our team! We are looking for a full-time RN to work day shifts Monday through Friday with clients throughout King and Snohomish counties. On-Call Duties shared with RN team include a rotation of weekends and weekdays.  Clients will primarily although not exclusively be in the geographic region that includes communities in King County and  South Snohomish County.

DUTIES INCLUDE:

  • Conducting client assessments
  • Developing care plans
  • Nurse Delegating, supervising and training caregivers
  • Reviewing records
  • Networking on behalf of CareForce
  • Geriatric Care Management

MUST HAVE: 

  1. A current and valid license to practice as a Registered Nurse in the State of Washington.
  2. Graduate credentials from an accredited School of Nursing; BSN preferred.
  3. Two years of home health or acute care clinical experience, experience with geriatric clients a must.  Experience as geriatric care manager preferred
  4. Ability to read and write English proficiently, ability to communicate with patients, families, personnel.
  5. A current and valid Washington State driver’s license and vehicle insurance.
  6. A working vehicle to visit clients in their homes.

COMPENSATION:Salary DOQ; benefit options include medical, dental, vision, holidays, paid leave time.

Applicant Requirements • Clear background check • CNA License, HCA License, RNA License with FOC and worked in 2011 • Current certification in First Aid and CPR • Excellent command of written and spoken English • Excellent professional caregiving references • Minimum of 6 months of professional caregiving experience • Proof of a TB test within the past year • Recent work experience as a caregiver.

 

If you meet the above requirements and you’d like to join our team of exceptional caregivers, please come into our Lynnwood or Tukwila office to take a skills test. Once you have passed the skills test you will fill out and application and get scheduled for an interview. Testing is held Monday through Friday from 9:00am to 3:00pm at the Lynnwood and Tukwila office.
