Care By The Visit

Are your care needs “brief” in nature?  A caregiver arrives at your home and in less than 2 hours performs tasks such as dressing, bathing, toileting, meal prep, and medication reminder.   The time of day you order your visit predicts the care needs you will have.  During a visit, the caregiver arrives, provides needed care, and then leaves for the day or until your next visit.

Caregiver arrives for short targeted care shifts (either 90 minutes or 2 hours in length):

  • Medication Assist
  • Dressing
  • Prepare and serve a meal
  • Bathing/shower
  • Toileting Incontinence Care
  • Prepare for Day
  • Prepare for Bed
  • Set up a lock box so caregiver can let themselves in/out if needed

 

Rise and Shine AM Visit – 90 minutes :  Licensed home care aide helps you awaken and get ready for your day.

Rise and Shine AM Visit – 2 hour visit : Licensed home care aide helps you awakeassist from bed for ads 4-06n and get ready for your day.

Meal and Companion Visit – 90 minutes : Licensed home care aide fixes you a warm, balanced meal and assist with any other care: lunch or dinner.

Shower or Bath Visit – 90 minutes : Licensed home care aide helps you with your shower/bath.

Shower or Bath Visit – 2 hours : Licensed home care aide helps you with your shower/bath.

Fluff and Tuck Visit- 90 minutes : Licensed home care aide helps you prepare for bed and can tuck you in over a 90 minute period.  Or a 2 hour visit to help you change into your bed clothes, help with toileting, bedtime snack, and medication assist.  Depending on how many tasks you need help with will tell you how long this visit needs to be.

CarePack®  A package of services to help you transition home from hospital or rehab.

Gold CarePack® Starter Service : The Gold Starter Pack helps you get home with support every day for 7 days.  With an RN visit on your first or second day home, two caregiver visits to help you with personal care/cooking/errands and care calls every day to make sure you are getting up, eating, and managing okay, this is the best support for your transition home.  Order for one week or supplement with our Gold Maintenance Pack.

Silver CarePack® Starter Service :Silver Carepack Starter includes RN visit, three ninety minute home care aide visits, and 5 CareCalls on days when the home care aide isn’t visiting.

Bronze CarePack® Starter Service :Bronze Carepack Starter includes RN visit, two ninety minute home care aide visits, and 5 CareCalls on days when the home care aide isn’t visiting.

Gold CarePack® Maintenance : After an illness, procedure, or hospitalization, order care by the week that is bundled.  Our mainenance package begins week number 2 after you have ordered the Gold Starter CarePack during your first week coming home.

Silver CarePack® Maintenance Service : Three caregiver visits during the week and 5 Care Calls provide 7 days per week of personal touch and support as you are recovering from your illness or accident.

Activity CarePack®:  Three visits per week with a home care aide to help you with your exercise program prescribed by your physician or physical therapists.  Our licensed aides can supervise walking, stretching, and most fitness programs.

Memory Impairment Safety Service – 90 visit : You create your activity goal and CareForce helps coach and guide you to achieving it: start small with increasing the number of steps you take in a day, or dream big, like joining a community exercise group.  You choose.  We help you realize your goal.  It’s based on the research evidence.

Memory Impairment Safety Service – 2 hour visit : Some people are safe at home alone but require a visit to be reminded to take medications and to have a balanced meal.

Care Calls : Some people are safe at home alone but require a visit to be reminded to take medications and to have a balanced meal. Experienced home care specialist calls you or your loved one to “chat” about your day, your medications, and any issue that is relevant to your situation.  You or your loved one meets with our clinical staff to develop a “script” for the care call to address important health and safety needs.  If a concern is noted during a care call, this will activate a call to clinical staff and family.
