Comfort Care

At some point in you or your loved one’s illness, whether you have enrolled in a Medicare end-of-life program or not, you may need care that focuses on your comforts rather than rehabilitation.  CareForce can provide you with excellence in caregiving by a very compassionate and skilled team of RN’s and home health aides to follow your desires in your care.  CareForce is known for our ability to work well with your Palliative Care or Hospice providers.  With you, your family, your physician and your other care team members, CareForce can help you discover the amount of care you need to keep you safe and comfortable.  Whether it is brief respite for your family caregivers or full time around the clock care so your family and friends can be your family instead of being your caregivers, CareForce is there for you in this time of need.  Most Medicare benefits do not include the hour by hour personal care that you need in between your Medicare team visits to manage your symptoms and needs. All serNurse Caring for Elder Patientsvices can be delivered as Comfort Care.  We have given you a few choices, just to inform you of what Comfort Care involves.

Free Nursing or Social Work Assessment and Plan of Care RN and/or  MSW assess your caregiving needs and develop a plan of care for CareForce caregivers. This is typically done in your home, but may be done at the hospital or skilled nursing facility before you go home.

Rise and Shine AM Visit – 90 minutes : Licensed home care aide helps you awaken and get ready for your.

Rise and Shine AM Visit – 2 hour visit:  licensed home care aide helps you awaken and get ready for your.

Meal and Companion Visit – 90 minutes : Licensed home care aide fixes you a warm, balanced meal and assist with any other care: lunch or dinner.

Shower or Bath Visit – 90 minutes : Licensed home care aide helps you with your shower/bath.

Shower or Bath Visit – 2 hours : Licensed home care aide helps you with your shower/bath.

Fluff and Tuck Visit – 90 minutes : Licensed home care aide helps you prepare for bed and can tuck you in.

Comfort Care Daytime Personal Care Service -6 hours : Qualified home health aide can provide all the comfort care needed whether you are receiving palliative care or are enrolled in a Medicare comfort care program.  Provides all aspects of care for 6 hours.

Comfort Care Daytime Personal Care Service -8 hours : Qualified home health aide can provide all the comfort care needed whether you are receiving palliative care or are enrolled in a Medicare comfort care program.  Provides all aspects of care for 8 hours

Comfort Care Daytime Personal Care Service –10 hours : Qualified home health aide can provide all the comfort care needed whether you are receiving palliative care or are enrolled in a Medicare comfort care program.  Provides all aspects of care for 10 hours

Comfort Care Daytime Personal Care Service – 12 hours : Provides all aspects of care for 12 hours during the day.

Comfort Care Nighttime Personal Care Service – 12 hours : You need sleep.  Hire a qualified home health aide can provide all the comfort care needed at end-of-life for 12 hours during the night. A trained and experienced aide will be awake and attentive to your loved one’s every need.

Comfort Care Nighttime Personal Care Service – 10 hours : You need sleep.  Hire a qualified home health aide can provide all the comfort care needed for 10 hours during the night. A trained and experienced aide will be awake and attentive to your loved one’s every need.

Comfort Care Nighttime Personal Care Service -8 hours : You need sleep.  Hire a qualified home health aide can provide all the comfort care needed for 8 hours during the night. A trained and experienced aide will be awake and attentive to your loved one’s every need.

Additional Comfort Care Hours : Add hours to our other personal care services to customize the length of your shift when you are enrolled in a Medicare end-of-life/comfort care Program.

Nurse Delegation – Washington State allows Registered Nurses to Delegate certain nursing tasks to clients who are considered medically stable and predictable, including end of life care. Click here to learn more about Nurse Delegation and the Law.

Nurse Delegated Daytime Personal Care Service – 12 hours : Qualified home health aide provides all aspects of care for 12 hours during the day including nurse delegated services.

Nurse Delegated Nighttime Personal Care Service – 12 hours : You need sleep.  Hire a qualified home health aide can provide all the comfort care needed at end-of-life for 12 hours during the night. A trained and experienced aide will be awake and attentive to your loved one’s every need. They then can also provide nurse delegated tasks such as medication administration.

Nursing Visit  : RN or LPN comes to your home to provide skilled nursing procedures.

Nursing Visit – IV Therapy : RN or LPN comes to your home to provide skilled nursing procedures.

Nursing Visit – Wound Care Management : RN or LPN comes to your home to provide skilled nursing procedures.

Care Calls : Experienced home care specialist calls you or your loved one to “chat” about your day, your medications, and any issue that is relevant to your situation.  You or your loved one meets with our clinical staff to develop a “script” for the care call to address important health and safety needs.  If a concern is noted during a care call, this will activate a call to clinical staff and family.
