Geriatric Care Management Consultation : One hour of time. Come to CareForce office in either Lynnwood or Tukwilla to discuss your care management needs with a certified or associate RN/MSW geriatric care manager.

Geriatric Care Management Comprehensive Assessment : Experienced geriatric care expert RN/MSW team assesses your care needs based on your personal goals.  A comprehensive plan is developed based on your goals for your consideration including medical, safety, housing, legal, social, and cognitive/psychological needs.

Geriatric Care Management Medical Assessment : Experienced geriatric care management RN/MSW team assesses your medical care needs based on your personal goals.  A comprehensive plan is developed for your consideration

Geriatric Care Management Safety Assessment : Experienced geriatric care management RN/MSW team assesses your safety care needs based on your personal goals.  A plan is developed for your consideration including the safety of your environment, your personal safety in your home and community, and the changes needed to help you have safe care or supervision based on your goals.

Geriatric Care Management – RN/MSW team assists you, ongoing with all of your care/living coordination needs. Coordinate support/care/medical appointments/information.  Communicate with family/legal representatives/physicians.
