01. WOUND CARE
Do you require simple or complex wound care and have no one to help you with this? CareForce RN’s and LPN’s can provide wound care in your home on days that your Medicare agency cannot come to your home or if you have been discharged from Medicare home health.
02. MEDICATION MANAGEMENT
Do you or your parent needs assistance with managing your medications? CareForce RN’s will communicate with your primary care provider, ensure that the medications that you are taking are the proper doses that are prescribed, monitor for side effects, and help you find a medication system that helps you take your prescribed medications as they are ordered. We visit once every 2-4 weeks to assess your ability to take your medications properly, help with ordering refills and setting up your medication system.
03. IV THERAPY
Did your doctor prescribe home infusion of antibiotics? Medicare or your insurance company pays only for the infusion company to deliver your supplies and to teach you or a family member how to administer the IV medication. CareForce RN’s or LPN’s can come to your home and administer those IV medications for you if you are unable or family is not able to administer all or some of the doses.
04. CHRONIC CARE MANAGEMENT
Persons with heart failure, chronic lung disease, or diabetes can benefit from an RN monitoring their vital signs and symptoms in their home. Our RN can go to doctor visits with you, communicate changes in your condition with your doctor or nurse practitioner, and help you discover strategies for keeping your symptoms under control.
05. LAB DRAWS
Persons who need assist with lab draws in the home.
06. NURSING ASSESSMENT
Skilled Geriatric Care Managers who visit you or your loved one in the home, and partner to create a detailed assessment for a guided care plan.
07. NURSE DELEGATION
Some nursing tasks can be delegated to credentialed caregivers. Tube feeding, medication administration (oral, topical, eye drops, and suppositories), and diabetes care are examples of tasks that RNs at CareForce can delegate to our caregivers to provide a less expensive alternative to hiring and LPN or RN to do these tasks for you in your home.
08. TRANSITION CARE
Being discharged from the hospital after a short or lengthy illness can be fraught with confusion and difficulty. For some people, their trip home is short lived because they have not properly transitioned to taking care of their recovery on their own. They end up back in the hospital. CareForce RN’s and Home Health Aides know how to help you transition to home safely with less of a chance of hospital return through medication reconciliation, teaching on how to manage your symptoms at home and coaching you or your loved one on strategies for recovery and comfort.
09. BLOOD THINNER MONITORING
Homebound patients who take blood thinners might require blood draws in the home to monitor their medication levels. CareForce RN's can come to your home to draw your blood and deliver to the lab for physician monitoring.
